Abstract
Giriş ve Amaç: Hospis bakım, yaşamın sonunda hastalar için kapsamlı ve konforlu bakım sağlarken, aile üyelerine de destek sağlayan bakımı içerir. Hospis bakım multidisipliner bir ekiple hastanın genel bakımını içeren ağrı yönetimi, ihtiyaç ve isteklerine göre uyarlanmış duygusal ve manevi destek hizmetlerinin sağlandığı bir ölüm sürecini kapsar. Modern anlamda ilk hospis 1967 yılında Cicerly Saunders tarafından Saint Christopher’s Hospice merkezinin kurulmasıyla başlatılmıştır. Diğer ülkelerde Güney Kore Seul’de 1965, Amerika Birleşik Devletler Connecticut’da 1974, Japonyada Yodogawa Cristian Hospital 1973, Almanya 1986, Çin’de Tianjin hastanesinde 1989 ve Danimarka’da 1992 yılında kurulmuştur. Türkiye’de hospis bakım daha çok kanserle mücadele noktasında başlatılmıştır. Türkiye’de 1993-1997 yılları arasında Türk Onkoloji Vakfı tarafından İstanbul Yeşilköy’de, hospis bakıma benzer içerikte bir “Kanser Bakımevi” adı altında bir merkez açılmış ancak bütçe problemleri nedeniyle kapanmıştır. Hacettepe Onkoloji Enstitüsü Vakfı tarafından 2006 yılında “Hacettepe Umut Evi” adı altında açılan merkez kanser hastalarının tedavi gördüğü ve kısa süreli konaklayabileceği bir yer olarak hizmet vermiş ancak bu kurum da kısa ömürlü olmuştur. Bu derleme dünyada ve Türkiye’de Hospis bakımın kapsamını belirlemek, mevcut durumunu incelemek ve geniş bir bakış ile değerlendirmek amacıyla yapılmıştır.
Sonuç: Yaşam sonunda bakımın yönetimi, anlamlı etkileşimler geliştirmek ve yaşam kalitesini korumak için destek evi sistemi içerisinde tasarlanarak hospis bakım yaklaşımları ile sağlanabilir. Ayrıca hospis bakım ile terminal dönem hastaların sağlık harcama giderlerini azaltarak ülke ekonomisine katkı sağlar. Türkiye için hospis bakımı uygulamak, bu konuda eğitilmiş personel yetiştirmek, hastalara yaşam sonu bakımını kaliteli bir şekilde sunmak ve bu imkanları sağlamak ihtiyacı vardır.

Abstract
Introduction and Aim: Hospice care includes care that provides comprehensive and comfortable care for patients at the end of life while providing support to family members. Hospice care involves a multidisciplinary team providing general care, pain management, and emotional and spiritual support services tailored to the patient's needs and wishes. The first hospice in the modern sense was started in 1967 by Cicerly Saunders with the establishment of Saint Christopher's Hospice Center. In other countries, hospices were established in Seoul, South Korea in 1965, in Connecticut in the United States in 1974, in Yodogawa Cristian Hospital in Japan in 1973, Germany in 1986, Tianjin Hospital in China in 1989, and Denmark in 1992. In Turkey, hospice care was initiated mostly in the fight against cancer. In Turkey, between 1993 and 1997, the Turkish Oncology Foundation opened a center called "Cancer Care Home" in Yeşilköy, Istanbul, with a similar content to hospice care, but it was closed due to budget problems. The center opened by the Hacettepe Oncology Institute Foundation in 2006 under the name of "Hacettepe Hope House" served as a place where cancer patients received treatment and could stay for a short time, but this institution was also short-lived. This review was conducted to determine the scope of hospice care in the world and in Turkey, to examine the current situation, and to evaluate it from a broad perspective.
Conclusions: The management of end-of-life care can be provided by hospice care approaches designed within the support home system to develop meaningful interactions and maintain quality of life. In addition, hospice care contributes to the national economy by reducing the health expenditures of terminal patients. For Turkey, there is a need to implement hospice care, to train personnel trained in this field, to provide quality end-of-life care to patients, and to provide these opportunities.
